• Medical and post-surgical care for conditions such as heart disease, orthopedic issues, diabetes, respiratory disease and cancer
• Medication management and education
• Chronic disease management
• Wound and skin ulcer management
• Rehabilitation and therapy after joint replacement, stroke or other neurological conditions 
• Home infusion services including injections and intravenous therapy
• Maternal/newborn and pediatric care, with emphasis on birth to age 3 
• Palliative care
